The FSAP online screening tools are available for confidential assessment of potential issues related to mental health and substance abuse. The self assessment tools, developed by Screening for Mental Health, are offered as a service through FSAP. The screenings are completely confidential and there are no fees. This online service provides you or a family member with the opportunity to assess common emotional and substance abuse concerns. Confidential results are provided immediately after each self assessment session.
The screening tools that are available include:
* Adolescent Depression
* Bipolar Disorder
* Eating Disorders
* Generalized Anxiety Disorder
* Posttraumatic Stress Disorder
* Alcohol Abuse
Please remember that the screening tools are meant to serve as a resource only not to diagnose mental health conditions or take the place of an in-person assessment with a Licensed Mental Health Professional. If you would like to schedule an appointment with one of the FSAP clinicians to review your results and/or for further assessment, please call 404-727-4328 during regular business hours. Please remember to bring a copy of your screening results with you to your appointment, as clinicians do not have access to your results or answers. These self assessment tools are available to support your understanding of conditions that may be impacting you.
The FSAP Behavioral Mental Health Team is available to assist you during normal business hours which are 8:00 a.m. - 5:00 p.m., and after hours for crisis support only by contacting our on-call service at 404-727-4328 and press #2.
